NOTICE OF REQUEST FOR PROPOSAL (RFP)
Title: Neighborhood Association Service Grant
Program Year: 2011-2012

RFP DUE DATE AND TIME: Monday February 28, 2011, 5:00 p.m. Online application ONLY.

SOLICITATION: Connecting Communities Foundation announces that proposals are being accepted for the 2011-2012 Connecting Community Needs with Resources Service Grant. It is estimated that the total value of the grant will be approximately $30,000. This service grant is a collaborative grant and will be handled in a two phased RFP process. The initial phase is for neighborhood associations to request service support for their designated neighborhoods. The second RFP is for direct service providers (nonprofits) to deliver requested services to the service grant awardees (neighborhood associations).
Connecting Communities Foundation is soliciting proposals from neighborhood associations who are desirous to receive services from direct service providers (nonprofits) that serve youth, families and neighborhoods within the boundaries of said neighborhood. The grant period to be funded is August 22, 2011 through June 30, 2012.
GENERAL DESCRIPTION: All neighborhood associations must be able to work with the collaborative network to produce outcomes that are measurable. Proposals may be submitted for any of the following objectives: 1) Crime Reduction, 2) Health, 3) Education, and/or 4) Employment. Proposals outside of these objectives will not be considered for acceptance. No proposal may request that funds be delivered directly to the neighborhood association, rather the funds will be used to contract direct service providers (nonprofits) in order to address the stated objectives and indicators through programs and services.

Proposers are responsible for checking the Connecting Communities Foundation website www.ccfneighborhoods.org for any addenda to the RFP and are required to read the entire Solicitation, including all referenced documents.
Proposals shall be submitted as defined in the RFP and in accordance with the Standard Terms and Conditions. Failure to do so may be cause for rejection as non-responsive. Late submissions shall be rejected.
